2018-2019 Online Registration Instructions

2018 - 2019 Online Registration Process
 

Please complete/save the following forms under the Registration Link on the school website:

Infinite Campus Link

  • Login to your parent portal
    • Edit any contact information you would like to change
  • Pay school fees
    • Go to the payment tab
    • Book & Technology Fees have already been assigned to students in grades K-12.
    • There is an Optional Items tab with a drop down arrow for each student. You may add a Student Activity Pass, Adult Activity Booklet, Padlock, High School Yearbook and all Early Learning Program Fees by clicking on your desired item(s) for each of your students.
    • Lunch deposits-you may set up Recurring Payments. To do this click on Add and you will have the choice to automatically deposit each month the amount you request on the date you request or you can set up to have a certain amount deposited into your lunch account each time your account gets to a certain balance. There are no convenience fees if you set up a recurring payment for lunch. You may also add money directly to your lunch account by going to the Food Service Account section and entering the amount you would like deposited.
    • After all of your fees have been added, click on the Continue button and Check Out. You must pay by E-Check. 

 
All forms are fillable PDF’s. Follow these instructions to complete and return:

  1. Click on the form you want to complete.
  2. If using Adobe Reader
    • Click on Print to Adobe PDF, Click Print
    • Go to File, Save as, and save to your desktop  - If saving Internet Usage Form - save as "Internet" to your desktop - this will make it easier to find later. Other file names to use FreeReduced, Health, Medication, Transportation, Field Trip, Counseling, Yearbook, BackPack.
  3. If using Adobe Acrobat DC
    • Double click on your download to open it (you may need to click on Show in Folder) each browser opens files differently.
    • Go to File, Save as, and save to your desktop  - If saving Internet Usage Form - save as "Internet" to your desktop - this will make it easier to find later. Other file names to use FreeReduced, Health, Medication, Transportation, Field Trip, Counseling, Yearbook, BackPack.
  4. Enter all of your information and Save.
  5. Next you will need to click here to upload your completed registration forms electronically. 
  6. Enter your first name and last name.
  7. Click on choose file and find the file you saved to your desktop and open it. This will attach your file. 
  8. Continue uploading all of your registration papers. You can attach up to 5 files. Then click on the submit button.
  9. If you have more than 5 files to attach, click here to upload more form. Repeat the process until all of your forms have been sent.
You may also print and return the forms to the school by scanning to mknipp@jesup.k12.ia.us or mail.

As Needed or Optional Forms:

  • High School Yearbook Form
  • BackPack Program – optional program that is available to all students
  • Free/Reduced Application and Fee Waiver – fill out if you would like to apply for free/reduced lunches and to waive or reduce school fees.
  • Medication Permission Sheet – only fill out if your student takes medication at school.
  • Transportation Form - only fill out if your student rides the bus
Passive Consent Forms:
  • Field Trip/Medical Consent Permission – only if you do NOT want your Middle School Student to attend field trips without parental permission for each trip or if you wish for your child to not receive immediate medical assistance on a field trip without advanced authorization (grades 5-8)
  • Small Group Counseling Permission Form only fill out if you do not wish for your son or daughter to participate in such a program.– K-6 grade students only
  • Release of Student Information/Olweus Parent Consent – read this information over.

 
Physical Form, Student Athletic Code of Conduct/Insurance Form, Concussion Form – Forms available on Athletic page. (Grades 7-12 only)
 
New students must go to the school to register. Registration will be held at Jesup Community School on Wednesday, August 8 from 9:00 am - 7:00 pm in the South Gym Lobby.
